According to a statement, the state Department of Community Health inspected a personal care home and issued arrest warrants for the owner and employees and ownership was temporarily revoked from current owners.
An investigation, also unveiled evidence of a long history of abuse with patients being slapped, restrained and tortured frequently by staff members. Also included in the investigation are three deaths that occurred earlier in the year.
Complaints go as far back as last 2012 which include missing medication, ventilation issues, failing to keep patients clean, employees with criminal convictions and allowing patients to wander away from the facility. For more, read the story.
