One state Department of Public Health has fined six nursing homes for various violations that endangered or injured patients.In one case, a patient developed a severe pressure ulcer. The patient who was incontinent was at risk for skin breakdown and developed an unstageable deep tissue injury in the lower back. Even though a pressure-reducing mattress was ordered for the patient, it was under-inflated on multiple occasions.
