A nursing home has been cited with allowing a patient death by failing to provide appropriate care to the patient's specific needs. The patient died of asphyxiation when his ventilator was detached. The patient was attending a church service and was left for an hour without ventilation, which he required to breathe and to live. A pastoral staff member heard the alarm but was told to ignore it.

A nursing home patient suffering from dementia has died after wandering from the facility in freezing temperatures. The patient was found dead in the courtyard just 30 feet from the door. The patient had been allowed to wander from the facility when her device did not set off the alarm.

The nursing home was rated with an “L”, the lowest ranking in deficiencies, citing immediate jeopardy for patients in the facility. Initial patients infected with scabies were not quarantined to prevent spread of the skin infestation.Nurses also contracted scabies from the patients and spread it to family members outside the facility.
