A nursing home closed after losing its Medicare and Medicaid reimbursements. The nursing home received countless violations of health and safety and housed the most sex offenders of any facility in the state. The state health department sent repeated letters of non-compliance when the facility failed to correct its health deficiencies.

A nursing home received the highest citation with a fine of $100,000. The citation involved a dialysis patient who was sent home from the hospital after surgery to be monitored. The nursing home failed to monitor the patient throughout the night after surgery and the patient was found dead the next morning.

A family recently settled a case with claims of wrongful death and negligence. The family’s loved one was a patient at a nursing home with a choking hazard.  The patient was left with a tray of food and found choking upon the nurse’s return.

A nursing home has been cited for multiple violations after a patient's mattress caught on fire. The facility director was unaware of the fire, until the fire department called to confirm the alarms indicated a fire. During the process of evacuation, the facility staff failed to count the patients to ensure all were safe as they were evacuated. Later the facility was cited for physical and verbal abuse of a patient for smoking on the porch.

A nursing home employee has been found guilty of lying to cover up responsibility of a patient fall. A patient fell, broke an arm, and suffered head injuries from the fall. The employee said she took a short bathroom break, but was seen on surveillance camera exiting the building.
