A nursing home has received two dozen citations in the past two years. Various issues can arise pointing to quality of care concerns for patients. Some of those care issues involve patients admitted for rehabilitation and receiving a lower level of care once their rehabilitation days are completed. “Hopefully, you’ve improved and don’t need a skilled level of nursing care anymore and can successfully manage at an assisted living facility,” said a local ombudsman.

A nursing home partial owner has been charged with theft. The nursing home executive stole $1,700 from a patient to purchase items for the nursing home facility. One of the items was a television.The charge involves felony dependent adult abuse-exploitation.

A nursing home employee has been charged with stealing from a patient. The employee stole a patient's credit card and charged items on the card at various stores in the area. The charges amounted to $700. The credit card was found in the nursing home employee's purse.

Two nursing home patients have died within months of each other raising questions about the level of care provided at the facility. One patient was allowed to be assisted by one person instead of a required two person assistance. The patient also was not provided with the proper lift device.

A state health department is overseeing the closure of a nursing home facility after it failed to improve treatment provided to patients. The nursing home facility was placed in probationary program that typically brings a facility back in compliance at the end of two years. The facility was not able to improve the necessary areas of care. As a result, the nursing home has decreased to 23 patients in a 54 bed facility.

Five nursing homes in the same state received fines after surveyors cited the facilities for various incidents. The incidents involved patients with dementia and bipolar disorder wandering away from the nursing home facility. One patient was not checked for 8 hours, when he should have been monitored every 15 minutes. Other patients were allowed to fall and suffer injuries to the head and eye socket .
