Inspection Reports for Nursing Homes in Mahoning County

In a recent report by the online newspaper Mahoning Matters,  hundreds of pages of inspection reports for t28 Medicare and Medicaid-certified nursing homes in Mahoning County were investigated. One noteworthy issue was Beegly Oaks Center for Rehabilitation and Healing, which was note with a much below average according to Medicare.gov, which gave the facility a one star rating. Rehab Center Fined $25,000 After Resident's Leg Was Amputated

An Illinois nursing home was recently fined $25,000 by the state in connection with a violation falling into the high-risk category. The Champaign Rehab Center failed to assess the resident's feet and obtain treatments for a resident with Type 2 diabetes and peripheral vascular disease, according to an Illinois Department of Public Health report from August 2019.    That failure resulted in the resident being admitted to the hospital with gangrene, which required an above-the-knee …
