The facilities were cited in connection with inspections between 2010 and 2012 by the State Department of Justice’s Operation Guardians. The State Department of Aging and Disability Services is investigating a nursing home after an patient at the facility disappeared and was found dead a week later. On July 8, police issued an alert that the 78-year-old Alzheimer’s patient was missing. Administrators at the facility told the man’s family they were searching for man, but his family had also begun searching on their own.
