State Investigates Nursing Home Care; Several Facilities Fined and Employees Dismissed
A registered nurse, who was employed at an adult care facility, has surrendered her license following an investigation by the State Office of Medicaid Inspector General. According to a detailed investigation report, the nurse verbally and physically abused patients. The report states that she forcibly held down one resident while providing care and called the resident an “evil witch.” The State Attorney General also investigated care at another nearby nursing home.

A 76-year-old female nursing home patient was transferred to Berkshire Health & Rehabilitation Center, a nursing home in Vinton, Virginia operated by Medical Facilities of America, Inc. The patient had severe respiratory disease on admission. The nursing home permitted the patient’s oxygen saturation to dip to 63%.
Nursing Home Allows Patient to Remain at Facility Only If Family Removes Hidden Camera
A 78-year-old nursing home resident whose abuse by nursing aides was caught on camera will be allowed to stay at the facility in exchange for taking down the camera. “It’s a victory, technically” said the woman’s son, “but it’s a victory for them because they got what they wanted. They got the cameras out of there.” Some states have laws in place that prevent a nursing home or its employees from covering or shutting off a video camera installed by a patient’s family to catch abuse.
Nursing Home Fined $100,000 After Patient's Neglect Related Death
A nursing home was fined $100,000 on Friday after state investigators determined that inadequate care led to the death of a resident, according to Department of Public Health officials. The investigation report reveals that staff at the nursing home failed to treat an elderly, wheelchair-bound patient after her left thigh bone broke mysteriously in October 2008. The serious injury caused her to suffer cardio-respiratory distress days later, according to the investigation.
Patient Fractures Shoulder in Fall from Hoyer Lift; Nursing Home Settles
An elderly female nursing home patient was totally dependent upon staff for transfers. The patient was transferred using a hoyer lift, which is a full mechanical lift that suspends the patient in a sling as the patient is transferred from one seating surface to another. During a transfer with a hoyer lift, the nursing home allowed the patient to fall and sustain an impacted fracture of her left shoulder.
Nursing Home Permits Patient to Fall from Bed; Jury Sides with Patient’s Family
An elderly female nursing home patient was permitted to fall from her bed to the floor. The nursing home claimed it was changing the patient’s bed sheets while the patient remained in bed. The patient was turned away from the aide who was making the bed and positioned on her side at the edge of the bed.
