A nursing home has been placed on a “Special Focus Facility” list following repeated violations and citations from state surveys. In the past two years, the nursing home has been fined more than $150,000 for failure to follow the standard of care for their patients. The facility is one of 86 nursing home facilities on the list.
The program associated with the list requires state inspections twice a year and stricter repercussions with failure to provide quality care to patients than other facilities that are not on the list. Past violations by the facility include failure to contain a patient with dementia who wandered away from the nursing home, failure to contain an aggressive patient that was violent toward other patients, and failure to detect dangerous levels of carbon monoxide.
