A nursing home has been cited for unsanitary conditions causing employees to speak out against the uncleanliness and poor care of patients. One nursing home employee filmed giant cockroaches in the kitchen and then reported it to administration. Rather than appreciation, the report was met with termination of the employee.

Other violations included pressure ulcers, failure to alert a doctor of a patient's chest pains leading to death, and a shortage of supplies to care for residents. For more, read the story.
