A 92-year-old patient was discharged from the hospital after a left hip fracture surgery on June 8, 2013 and admitted to the nursing home for therapy. The patient was receiving Lovenox and Coumadin daily, with Admission Orders to the nursing home to maintain INR in the 2-3 range and to check her PT/INR daily until discontinued. The patient was identified as being “at risk for bleeding/bruising/abnormal labs” related to the Coumadin and Lovenox.

A 73-year-old male nursing home patient had a high fall risk and the danger of sustaining injury during sliding board transfers, of which the nursing home was aware. On November 7, 2012, the patient fell at the nursing home facility when a single CNA was transferring him using a sliding board. On September 17, 2009, an 87-year-old female nursing home patient began having loose stools. Over the next two months, the patient continued to suffer from C.diff diarrhea, which the nursing home failed timely to diagnose. The nursing home improperly administered Imodium to the patient in response to her diarrhea.During the time of care, the patient was also a high fall risk based on her history of falls, unsteady gait, muscle weakness, use of assistive devices, diuretic medications, cardio…

An 81-year-old patient was admitted to a nursing home on March 18, 2009. On admission, he was known by the defendants to be a high fall risk due to an unsteady gait and weakness and poor safety while ambulating, among other medical conditions and limitations.On March 21, 2009, the patient was permitted by the nursing home to fall in his bathroom. The patient was found in the bathroom floor.

A nursing home permitted a 73-year-old female patient in their care to fall. On November 20, 2007, the patient was documented by the nursing home to have been found in her bed crying, complaining of right hip and leg pain. Her leg was also shortened.

On December 8, 2012 at a local adult training center, a 57-year-old male was improperly transferred in his room, resulting in a fracture of his right ankle. The patient required the assistance of two staff for all transfers. Unfortunately, the nursing home aide responsible for his care transferred the patient by herself.

An 83-year-old female was admitted to a nursing home on June 14, 2011. At all times relevant, the nursing home knew the patient was a high fall risk. Unfortunately, the nursing home failed to provide the patient with the safety measures they knew she needed, including without limitation vigilant supervision and bed and chair fall alarms.On June 14, 2011, within an hour of admission to the nursing home, the patient was left unattended and unsupervised in her room.

A 74-year-old patient was admitted to a nursing home’s care on January 12, 2008. The patient was nonverbal, had lower extremity weakness due to a previous stroke, and was non-ambulatory. The patient was totally dependent on the nursing home facility for ambulation.On June 8, 2008, the patient was documented as being agitated and “climbing OOB (out of bed).” The physician was notified and an order for Ativan was obtained.

On January 31, 2007, a 95-year-old female was admitted to a nursing home. The nursing home knew the patient was a high fall risk based on a medical history that included a history of falls, dementia, the use of assistive devices, osteoarthritis, hypertension, dizziness, problems with balance, and confusion, among other conditions and limitations.On February 6, 2012, the nursing home permitted the patient to fall when a CNA was putting her to bed. A 91-year-old female patient sustained a fall at home on July 3, 2013. As a result of the fall, she was hospitalized at the hospital with left hip and left wrist fractures. She received surgery at the hospital to repair the fractures.
